Predictors of development of cardiovascular events after stenting of coronary arteries at patients with obesity

Abstract

Aim. Study predictors of development of cardiovascular events after stenting of coronary arteries at patients with the coronary heart disease (CHD) with obesity.Material and methods. The study population included 186 (54,4±9,1 years) patients with coronary artery disease (CAD) and obesity undergoing percutaneous coronary intervention (PCI) with bare-metal stenting and at least 12 months of follow. The all patients research main and additional metabolic risk factors and epicardial fat thickness (EFT) with determination of echocardiography. Cardiovascular events were registered at 58 (31%) patients. The lipoprotein a - LP(а), leptin, blood glucose, interlaken-6, diastolic arterial pressure and index prevalence of coronary atherosclerosis on Gensini scale became predictors of cardiovascular events. The indicator of EFT showed interrelation with risk of a cardiovascular events. Conclusion. Determination of additional parameters of a lipid profile, carbohydrate metabolism, pro-inflammatory activity of plasma, neurohumoral activity of visceral fat and degree of epicardial adiposity is necessary for the purpose of formation of group of high risk of complications and carrying out prevention of risk factors before carrying out a coronary revascularization.

The role of amlodipine (norvasc) in Cardiology

Abstract

The review concerns amlodipine - the representative of the third generation of dihydropyridines. Amlodipine, being considered a hypotensive drug, has a broad spectrum of clinical effects beyond the hypotensive effects. It is shown that the drug has different pleiotropic effects, allowing using it as an anti-atherosclerotic and organoprotective funds. The data on the beneficial effect of amlodipine in pathology, is not included in the group of cardiovascular diseases - Raynaud's syndrome, migraine headaches, tendency to vomit.

The mechanism of action of metabolic cytoprotector trimetazidine in acute ischemia-reperfusion injury

Abstract

The review presents, as the classical data on the mechanism of action of metabolic cytoprotector trimetazidine in acute ischemia/reperfusion injury associated with a partial inhibition of the oxidation of long chain fatty acids and increased metabolism of pyruvate, as well as new concepts of reducing the level of oxidative stress, reduction of cardiomyocyte apoptosis, elimination areas of myocardial stunning and hibernation state. Described cytoprotective effects associated with inhibition of activation of mitochondrial pore with transient (temporary) permeability. Presented clinical studies showing significant anti-anginal and anti-ischemic effect of the trimetazidine in patients with stable angina, to decrease myocardial damage in acute coronary syndrome, during intervention on the coronary arteries. Particular attention is given to the latest data on the positive effects of prolonged use of trimetazidine on the course and prognosis in patients with heart failure.

The functional significance of vasomotor arteries dysfunction in the clinical assessment of myocardial ischemia in case of carbohydrate metabolism disorder

Abstract

We investigated the relationship between the severity of silent myocardial ischemia and the functional state of arteries endothelium. We studied the significance of violations of vasomotor function with the loss of the ability to flow-dependent vasodilation as a risk factor, which increases the probability of the episodes of silent myocardial ischemia among patients with Type II DM. We observed 128 patients (66 male and 62 female), average age was 59,3±4,7. Each patient signed an agreement to take part in our research as a volunteer. The agreement was adopted by the local ethics committee.Patients with coronary heart disease and Type II DM had dominating daily duration of episodes of myocardial ischemia and a maximum depth of ST segment depression. Besides, they had reduced endothelium-dependent vasodilation reaction (EDVD=3,7±1,1%). The violation of the functional state of the vascular endothelium correlates with the registration frequency (R=-0,68; p <0,05) and the duration of episodes of silent myocardial ischemia (R=-0,53; p <0,01).

The role of HIV-infection in the development risk factors of cardiovascular diseases in naive HIV-infected patients and HIV-infected patients on antiretroviral therapy

Abstract

Cardiovascular diseases are one of the main reasons of death in the modern world. Risk factor conception is emphasized in theirs prophylaxis. Nowadays besides risk factors mark out so called «special» population groups in which prophylactic actions school be more aggressive because of high cardiovascular risk. One of such groups is HIV-infected patients. It well known that among them the prevalence of traditional and «new» risk factors is much higher. At the same time in spite of the fact that there are a lot of trials it is lack of evidence about cardiovascular risk factors in HIV-infected patients with the saved immune system function as in «naive», so in antiretroviral treated patients. The aim of our investigation was to evaluate range of express traditional and «new» risk factors in «naive» and antiretroviral treated HIV-infected patients with the saved immune system function.

Application of lercanidipine in the treatment of hypertension

Abstract

The article presents data from clinical trials that have examined the efficacy and safety of modern calcium antagonist (CA) III generation of lercanidipine. The results of studies show that treatment with lercanidipine leads to a marked reduction in systolic and diastolic blood pressure without significant effect on heart rate. Unique pharmacokinetic characteristics of the drug provide its excellent efficacy and safety, good tolerability, convenience, acceptance and high adherence CA data in patients with hypertension. Additional properties of lercanidipine allow us to recommend the use of the CA in comorbidity.
